Appointments
We see patients by appointment in order to allow the doctor sufficient time to address your health care needs. We ask that you arrive 15 minutes before your scheduled appointment time so that you can complete any paperwork that may be required. Please bring your insurance card(s), photo ID, as well as your medications or a list of your current medications.
Parents or legal guardians must bring children to Well Child appointments due to signature requirements when immunizations are administered.
We ask that you call our office at least 24 hours before your scheduled appointment time if you need to cancel or reschedule. This gives us the opportunity to see other sick patients who needs an appointment. Patients who repeatedly miss appointments may incur a missed appointment charge.
If you are late for your appointment, it may be necessary to reschedule your appointment for another time. Therefore, you should call the office if you foresee any delay in arriving on time for your appointment.
Medications
Please bring your medications or a list of your current medications with you to each office visit. For prescription refills, please call your pharmacy; they will contact our office to approve your request. Remember, there are some medications that cannot be called in to the pharmacy. It may be necessary for you to come to the office for a written prescription. Long-term usage of certain medications may require regular office visits.
Insurance/Payment Information
We accept most insurance plans including Medicare and Medicaid. Please remember the insurance company's contract is with you. You are responsible for a co-pay as well as a deductible. It is your responsibility to make sure your visit or services provided are covered by your insurance plan.
We accept payment by cash, check or debit/credit card (Visa, MasterCard, Discover). If you choose to mail your payment, please make your check payable to Quail Ride Family Practice and include your account number on the memo line. Payments can be mailed to:
